Medically Fragile Homeless

The Medically Fragile Homeless (MFH) program provides housing for medically fragile persons in San Luis Obispo County who are referred by an agency, organization or hospital with a medically verified need for non-congregate 24/7, temporary housing.  Housing may be provided from 3 to 90 days dependent upon need and availability.

The primary goal is physical recovery without readmission to the hospital.  MFH also provides shelter to persons who have need of housing to enter a hospice care program where the goal for the person is to die in the dignity of housing with the care of hospice.

The program does not provide medical care to those who are housed but ensures that the person has access to meet their basic needs, including transportation to medical appointments.  The referring agency is responsible to ensure any follow up medical care or in home services that are needed.

MFH provides supportive services and coordinated case management to ensure physical recovery, access to income sources, connection to services and a more permanent housing alternative at exit of the program.  MFH works with all existing case managers to ensure coordination of goals, services and follow through.
